WHAT IS THE CENTER FOR RECONSTRUCTIVE URETHRAL SURGERY?

The Center for Reconstructive Urethral Surgery is the first worldwide center entirely devoted to the diagnosis and treatment of male urethral stricture and genitalia diseases linked to a urethral pathology, such as lichen sclerosus or failed hypospadias. The Center is also involved in the treatment of male urinary incontinence with the use of artificial sphincter AMS 800, as well as the treatment of congenital penile curvature.

The Center is active on primarily four fronts:

Clinical, diagnostic and therapeutic activity

The Center for Reconstructive Urethral Surgery provides all necessary clinical and radiological tests that allow for correct diagnosis in patients suffering from urethral stricture, urinary incontinence, lichen sclerosus, failed hypospadias, and congenital penile curvature. The Center guarantees a high therapeutic level, as it offers the patient the possibility to receive treatment using all types of surgical approaches known to date in treating the above-mentioned pathologies, as well as a customized care path aimed to achieve the best compatibility with the individual.
The Center also offers the possibility of receiving treatment at no additional cost to the patient who is covered by the Italian National Health System, so to provide a service of the finest quality and accessibility to the greatest number of people possible. As mentioned before, our activity as well as our efforts are all patient centered; following surgical procedure, each patient is guaranteed a place in our exclusive follow-up program, thus receiving qualified life-long coverage.
Witness to the excellence of our Center and the quality of the objectives reached, in terms of diagnostic, surgical and therapeutic activity, we have today the pleasure of assisting more than 400 patients coming from Italy and all over the world, while focusing on achieving our main goal: patient quality of life and overall health.


Research activity

Along with a high clinical standard, the Center is also engaged in clinical research. This we feel is what provides the Center’s competitive edge globally. Thanks to the combination of clinical research and patient care, over the last years, a number of innovative surgical techniques have been developed and validated and the results translated to the patients.
The Center for Reconstructive Urethral Surgery is one of the first in the world to use oral mucosa for urethral reconstruction, as well as fibrin glue and minimally invasive techniques. The quality of our clinical activity has been translated into scientific knowledge. Right from the start, we have emphasized the severe potential risk of urethral involvement in patients suffering from lichen sclerosus and have dealt with the challenge of failed hypospadias repair.
Currently, the Center for Reconstructive Urethral Surgery is involved in a European project on tissue engineering. As soon as possible, in our Center will be available  new techniques for urethral reconstruction using material from laboratory of tissue engineering. Together with two of the most prestigious urological schools worldwide (London and Hamburg), we are addressing the diagnostic and therapeutic challenges of the near future. We’ll be dealing, firstly, with the issue of the “patient’s reported outcome”. We are working toward developing a very specified questionnaire in order to better understand the patient’s expectation and perspective.
Another research issue is investigating and studying post-surgical complications in patients who have undergone urethroplasty, in order to reduce the seriousness of these complications with increasing efficacy and swiftness. Our long-term target is to register a significant increase in the present success rate of urethral surgery, bringing it to the extraordinary rate of 90% - more than a 10% increase. Our studies look very promising, and we can already guarantee these results for some types of surgery, and current data are increasing our hope of making an additional improvement.   


Scientific activity

The high number of articles published by the world’s most influential and relevant Urology journals each year and by exclusive invitations to world-class and prestigious Urology Congresses, the most rewarding prize for our hard work, bear witness to the scientific production of our Center.
In recent years, we have been receiving more and more invitations to participate in international meetings and live surgery courses, where we are able to present the specific techniques we have applied in our Center to track treat challenging urethral diseases.


Teaching activity

Our Center stands out for its key role in educational programs aimed to improve the individual professionalism of our applicants and Fellows in urethral reconstructive surgery. The Center is part of the International School of Urethral Surgery and offers along with the other two Centers in London and Hamburg, high level training prograns in urethral surgery. Every year, we receive many international students desiring to dedicate their time and their professionalism to urethral surgery.
One of the main goals of our mission is to help physicians who live in developing countries desiring to improve their theoretical and technical skill in the field of reconstructive urethral surgery. Within the context of this program, we have already developed fruitful collaboration with several countries ((India, Mexico, Armenia, Egypt, Serbia) in order to implement the creation of a specialized urethral Center in each nation.


Humanitarian activity

The Center, in collaboration with others urologists, founded the HuRGUST (Humanitarian Reconstructive Genito-Urinary Surgeons Team).
The aim of the group is to deliver, in developing countries, high professional surgical treatment to children and adults suffering from:

  • Bladder exstrophy and complications after surgery for bladder exstrophy.
  • Epispadias and complications after surgery for epispadias.
  • Hypospadias and complications after surgery for hypospadias.
  • Congenital malformations of male and female genitalia and complications after surgery for these malformations.
  • Trauma of male genitalia and complications after surgery for these trauma.
  • Urethral trauma and and complications after surgery for these trauma.
  • Urethral strictures and complications after surgery for urethral stricture repair.
  • Other congenital or acquired diseases of male and female genitalia in children and adult.

The Humanitarian Reconstructive Genito-Urinary Surgeons Team (HuRGUST) members will deliver their care as volunteers and they will provide the basic instrumentations for the main surgeries. Members will provide care for a maximum of 4 days in the countries after the official request. A particular interest will be directed to teaching.

Here you can find information on all of the techniques we perform, along with all surgical results and all complications recorded. The web site is updated monthly. For each technique, you can find pictures and drawings, as well as movies of the principal reconstructive operations. Many free, downloaded pdfs allow the physician and the patient to gain an idea of the activity performed and to learn about the long-term anatomical and functional outcome for each type of surgery.
